SECTION 1
At the heart of each and every one of our thoughts, decisions, actions and reactions lies a set of beliefs, ideas, and opinions which influence them. These are our values, and these are shaped throughout our lives by our own individual experiences. In the rest of this section we’ll look at the values specific to Person-Centred Care and how to work in partnership with the people you care for in order to plan a service tailored to their unique needs, wishes, and preferences.
SECTION 2
Before you’re able to offer your patients person-centred care which is tailored to their individual needs you first need to find out what those needs are. This section is all about how you can support your patients in a person-centred way. We’ll look at the Care Plan; we’ll consider ways of recognising and minimising sources of discomfort and pain; and then we’ll see what you can do to can help your patients maintain their sense of identity and self-esteem.
